Starter Intake Through Google Forms
The website’s starter intake buttons open a Google Form. Responses are collected through Google Forms for Izzy-A-Nanny to review and contact you. They are not automatically copied into this website’s client portal or database. Google’s services and privacy policies apply when you use the form.
Existing Website Portal Records
For families who use the existing website portal, records may include their sign-in account identifier and email, parent or guardian name, contact details, child’s age, city, session and activity preferences, preferred timing, and contact method. The portal also stores intake approval status, scheduling requests, and booking decisions.
How Information Is Used
Izzy-A-Nanny uses this information to respond to your request, review whether care may be a fit, organize intake, and manage approved-client scheduling. The business owner can view submissions. Signed-in families can view their own records.
Keep Sensitive Details for Full Intake
Please do not include medical records, diagnosis details, medication information, full child names, or intimate personal-care information in the starter form. We will discuss the appropriate process for detailed intake directly.
Storage and Access
Google starter form responses are stored through Google Forms. Existing website portal records are stored in the site’s hosted database, and portal sign-in is provided through ChatGPT. Scheduling access is granted only after business approval. The starter site does not process payments or send automated marketing emails.
